Foreign passport

If the passport is lost / stolen, or its validity has expired, and there is a need for an urgent trip to Ukraine, we recommend issuing an identity card to return to Ukraine.

How to issue a new passport

Step 1: Be a citizen of Ukraine permanently residing or temporarily staying in the consular district

Consular District Borders: Idaho, Arizona, Alaska, Wyoming, Washington, Hawaii, California, Colorado, Montana, Nevada, New Mexico, Oregon, Utah

Step 2: Have the necessary documents according to the list. The list is contained in the corresponding section on this page

Step 3: Arrive at the Honorary Consulate of Ukraine in Seattle and submit the documents in person. Reception of documents is carried out without prior registration in the order of the live queue according to the schedule of reception of visitors.

  • If a person is over 16 years old, the personal presence of this person is mandatory and the documents are submitted personally by this person.
  • If a person is under 16 years of age but over 12 years of age, the personal presence of this person is mandatory and the documents are submitted by one of the applicant’s parents
  • If the person is under 12 years of age, the personal presence of this person is not required and the documents are submitted by one of the applicant’s parents

Step 4: Wait for the passport to be issued and delivered

Step 5: After delivery of the issued passport, arrive at the Honorary Consulate of Ukraine in Seattle and personally receive the issued passport

The passport is issued at the Honorary Consulate of Ukraine in Seattle only in person to the applicant. A passport of a person under the age of 16 may be obtained by one of the parents (adoptive parents) of this person, provided that a passport or other identity document of the recipient is presented.

Upon receipt of a new passport, you must provide a previous passport.

If the passport was issued for the first time, or the previous passport is in the Honorary Consulate of Ukraine in Seattle, you must provide proof of identity. If the previous passport has been lost / stolen, a police report must be provided confirming the applicant’s complaint about the loss / theft of the passport and an identity document.

Necessary documents for a new passport

Document 1. Questionnaire. The application form is completed. Sample filling

Document 2. Photo

  • For a person under 12 years of age – 1 color printed photo of 10 x 15 cm with a clear image of the face on a white background (the face should occupy 80% of the photo)
  • For a person over 12 years of age – photography is carried out directly at the Honorary Consulate of Ukraine in Seattle, no additional fee is charged

Document 3. Passport

  • Passable foreign passport
  • Copies of passport pages with entries must also be attached

In case of absence of the passport, the following is submitted:

  • Police report – in case of loss or theft of the passport – the original police report stating that the applicant’s passport was lost / stolen. Additionally submitted: a copy of the lost passport (if available), a copy of the internal passport (if available).
  • Travel document of the child, or passport of one of the parents, where the person is registered – if available (if the person issues a passport for the first time).
  • Certificate of acquisition of Ukrainian citizenship – if a person was born abroad (in the US, etc.) and issues a passport for the first time
  • A copy of such a document must also be attached

Document 4. Parents’ permission to issue a passport

  • Only for a person under 16 years of age – The application is submitted by one of the parents who does not submit documents and does not fill out a questionnaire for a child’s passport and has Ukrainian citizenship. If you do not have the citizenship of Ukraine and have the citizenship of another state – a copy of the passport confirming the affiliation to foreign citizenship is provided.
  • In case of absence of one of the parents during the submission of documents, the relevant application is submitted notarized

Document 5. Birth certificate + copy – submitted only for a person under 16 years of age. Birth certificate issued in the USA (or in another foreign country) is submitted with a certified stamp “Apostille” and with a notarized translation into Ukrainian.

Document 6. Proof of stay in the United States

  • This can be one of the similar documents: “Green card”, Driver License, ID, valid visa, form I-94, certificate of employment, rental agreement, certificate of open a bank account, etc.
  • A copy of such a document must also be attached
  • The last name and first name in such a document must match the last name and first name on the passport.

Document 7. Marriage certificate, divorce certificate, certificate of change of name or surname, or a relevant court decision – in the case of a passport for a new surname or name.

The relevant document must clearly indicate the transition from one surname / name to another. The document, issued by the competent authority of the United States, is submitted certified by “Apostille” and with a notarized translation into Ukrainian.

WARNING! You can issue a new passport with a new surname if:

  • the person has issued a permanent residence permit abroad

or

  • the person has issued an internal passport (passport of a citizen of Ukraine) with a new surname (a copy of such passport is attached)

Document 8. Consular fee + copy

$ 40 – for a passport for a person under 16 years of age
$ 130 – for a passport for a person over 16 years of age

WARNING! The consular fee is submitted in person together with other documents!
A copy of such money order must also be attached.

The consular fee is paid in the form of “Money Order“. Cash, credit cards, checks, bank transfers are not accepted!

Type of issued passport

  • Passport of a citizen of Ukraine for travel abroad (Biometric passport)
  • Transliteration check
  • Validity of the issued passport:
    10 years – for persons who have reached 16 years of age.
    4 years – for persons under 16 years of age.

It is not possible to enter data on children in parents’ passports. Every citizen, regardless of age, can get a separate new passport.

The Honorary Consulate of Ukraine in Seattle does not transfer visas or other marks of foreign states to the passport.

Checking the status of the passport

To check the status of the passport, go to the link passport.mfa.gov.ua and enter the session ID, which was provided to you when applying for a passport.

  • Data sent for personalization – means that all checks have been carried out and your personal data has been sent to the printing plant for passport printing.
  • The document has been prepared – it means that the passport has already been printed, but has not yet been delivered to the Honorary Consulate of Ukraine in Seattle.
  • The document has arrived at ZSU – means that your passport has arrived at a foreign diplomatic mission, ie the Honorary Consulate of Ukraine in Seattle, and you can get it.

The term of registration (excluding the terms of delivery of the finished passport to the Honorary Consulate of Ukraine in Seattle from the Ministry of Foreign Affairs) is up to three months from the date of submission of documents.
