A person whose parents had Ukrainian citizenship at the time of her birth.
The foreign diplomatic mission to which the documents on registration of acquisition of Ukrainian citizenship are submitted shall check the compliance of their registration with the requirements of the legislation of Ukraine and confirm the existence of facts to which the Law connects a person’s citizenship of Ukraine.
The decision to issue a person acquiring Ukrainian citizenship or to refuse to satisfy the application shall be made by the head of the foreign diplomatic mission or his deputy not later than within one month from the date of receipt of the documents. The decision to refuse shall be notified to the applicant in writing no later than one week from the date of its adoption.
If a positive decision is made, the person is registered as a citizen of Ukraine and documented by a certificate of registration of the person as a citizen of Ukraine, which is submitted by him to obtain documents confirming the citizenship of Ukraine.

2. Original birth certificate
The original certificate issued in the United States must be certified by a special “Apostille” stamp affixed to the Office of the Secretary of State.
The original translation of the child’s birth certificate into Ukrainian is also provided (all pages of the certificate and apostille are translated). The translation must be notarized and certified by a special “Apostille” stamp affixed to the Office of the Secretary of State. (If the translation is made and notarized in Ukraine, Apostille certification is not required).
3. Document on change of parents’ surname (if necessary)
Document on change of the surname (name) of the father / mother, if their surnames in the passport and in the child’s birth certificate are different and a 2-sided copy of all pages of the document. The original of the document issued by the appropriate competent authority of the United States must be certified by a special stamp “Apostille” affixed to the appropriate competent authority of the United States (Office of the Secretary of State). The document is submitted apostille together with a notarized professionally executed translation into Ukrainian (all pages of the document and apostille are translated). If the translation is made abroad, it must also be certified with the stamp “Apostille”. The original document is returned to the applicants after acceptance of the documents, a copy is attached to the case;
4. Passport
A copy of a valid foreign passport of one of the parents, confirming the fact that one of the parents of the person is a citizen of Ukraine at the time of his birth.
